(21.10.13) Flu Clinics

This year's flu clinics are being held in October.  If you are over 65 or in one of the "at risk" groups and have not received your vaccination, make an appointment to see the Nurse.


(02 02 12) Sickness Certificates

For the first week of any illness you should use the SC2 self certification form.  These forms are no longer available at reception.  Click here to print a copy of the SC2, once it has been completed take it to your employer.

If your illness lasts longer than one week, you need to see a doctor regarding further certification.



From 1st June 2010 the Out of Hours telephone number will change to 028 3839 9201.

New Patient Registrations

You will be asked to complete new patient registration forms.  Print and complete the forms below:

  1. New Patient Registration  
  2. Medical Details
  3. Ethnicity Questionnaire

Please bring the completed forms to reception together with:

  1. Your medical card.  This must be signed by you - or 
  2. If you have lost your card or have been outside of the UK for more than twelve months you will need to complete a HS200 form (available from reception) or Click here to download a form.
  3. If you are registering in the UK for the first time, you will be asked to complete a form HS22X (available from reception) or click here to download a form.

Depending upon your medical history you may be asked to attend a registration appointment with a nurse or doctor.
